In the p[ar]yshe churche of Chepingnorton beynge sycke of bodye & hole of

mynde make my last wyll & testament in man[ner] & forme folloynge fyrst

I beqethe my sowle unto allmyghty god unto o[ur] lady saynt mare

& my bodye to be buryed in the churche yarde of Chepingnorton by
